You will be surprised to know, but it is a fact, that lower back pain (LBP) is the second only to common cold as the most frequent reason for visiting any physician and most common chronic pain in industrial countries. Most commonly, people aged between 40–60 years are the mostly affected by it.
Ayurveda calls it, as prisht shool. It is condition generated by abnormal vata. Because of any injury, tumor, stretch like external factor or due to certain internal factors vata starts migrating towards back and due to its properties causes pain.
Back pain be classified into three categories.
- Acute - it lasts less then 6 weeks
- Sub-acute - it lasts between 6-12 weeks
- Chronic - it lasts more than 12 weeks
Mostly back pains does not signify any serious problem and resolves it self in few days or week.
Causes can be divided into two categories: -
1. Congenital- abnormality in anatomy of back bone, example kyphosis or stenosis
2.
Acquired- it can be categorized into following types: -
- Inflammation - It can occur due to infections like: -
a) Osteomyelits e.g. tuberculosis
b) Discits
c) Epidural abscess
It may occur by any other cause other than infection like in ankylosis spondylitis.

Trauma – any kind of injury or trauma to ligaments, muscles and vertebrae

Vascular – rupture, erosion of adjacent structures, epidural haematoma, dissection etc.

Neoplasm – primary tumors of spine – Multiple myeloma Metastic
- Degeneration – Disc disease, facetal arthropathy, spinal canal stenosis
- Metabolic – Osteoporosis, Paget’s disease
- Mechanical – Poor posture aggravated by obesity pregnancy and overuse
- Referred pain-
a) Abdominal viscera e.g. renal, pancreas, posterior duodenal ulcer
b) Pelvic disorder e.g. pelvic inflammatory disease (PID)
- Psychogenic factors – these can be the important factors
- Miscellaneous- whole body vibration
Lower back pain of any sort involves spasm of the muscles present along the spine.
(caused by sprain in muscle)
- Low back pains some where between bottom of ribs, back and tops of legs.
- Pain often begins suddenly or over a longer period of time
- Muscle spasm, Cramping and stiffness are often present.
- It is aggravated by weight bearing or specific movements and is relieved by rest.
- Pain may get worse on turning twisting even by coughing or sneezing.
SYMPTOMS (caused by nerve root pressure)
- Leg Pain-if pain extends below the knee, it is more likely to be due to pressure on nerve then to muscle problem. Most commonly, the pain starts in the buttocks and travel down the legs as far as the ankle or foot. This kind of pain is commonly known as sciatica.
- Nerve related problem such as tingling, numbness or weakness in one leg or in foot, lower leg or both legs. Tingling may begin in buttocks and extend to the ankle or foot. Weakness or numbness in both legs, along with loss of bladder and bowel control is symptoms of cauda equina syndrome.
- Least pressure should be exerted on back
- Use of lumbar belt should be encouraged.
- Bed rest should be done to avoid any unwanted stress on back
- While sitting always try to have a beck rest
- Hard bed is advised
- Massages should be done to reduce muscle sprain
- Hot fomentation will be very useful in back troubles
- Excess walking should be avoided
- Forward and backward bending should be avoided completely
- Garlic mixed with guggulu should be taken 3grams twice daily with warm water
- Jaggery mixed with dried Amla is made into decoction and should be taken 15-20 ml daily with equal amount of water
- Errand oil for local application is quite effective
- Rasna powder5grams to be take twice daily with luke warm water
